What We Do
Community Residential Unit (CRU)

The Heartland Center for Behavioral Change (HCBC) has provided residential services to the State of Missouri Department of Corrections, and the State of Missouri Department of Mental Health for over 35 uninterrupted years. Heartland’s experience with the Jackson County Drug Court and the City of Kansas City Municipal Treatment Court has enabled our agency to establish a Community Residential Unit (CRU). Other community funders that utilize the community Residential Unit include Clay, Platte, Ray County Mental Health Levy Board, COMBAT (Community Backed Anti-Violence Tax), and a Missouri Department of Corrections Re-Entry Grant. Heartland Center believes in assisting people in building healthier, happier, and more productive lives. HCBC would like to offer a clean, safe, residential environment for individuals who are in need of shelter. HCBC has experienced staff and can provide residential services to include food, shelter, basic recreation and access to laundry facilities.
Eligibility:

Adult men and women

Person’s able to provide for their own basic care including medication management, wound care, basic personal hygiene etc.

All residents must be reasonably stabilized relative to any chronic, serious medical or mental health conditions including having prescribed medications and access to all necessary medical and mental health medications and supplies.

All residents should be free from any known contagious conditions, including COVID – 19.
Services:

Accountability measures to include 24 hour a day trained staff on site who conduct rounds, observe self-administration of secured medications, head counts of all residents conducted 6 times per day, and a system for accountability for passes outside the building based upon need and with coordination of the referent.

Heartland Center can offer access to transportation as needed.

Residential services with living space with at least 48 square feet of living space per resident.

Onsite food service for consumers to access at designated mealtimes.

Television and recreation space for utilization for men and women in separate areas of the facility

A gymnasium to utilize for recreational activities to promote health and well-being for residents.

Collaboration for medical and mental health services with local providers.

Care Coordination to ensure access to needed services with a goal if personal independence and self-actualization. This service will address needs around medical, mental health, employment and stable housing.
